This may only apply to roughly 37 people on the MacRumors Forums, but out of curiosity, I plugged my Identiv SCR3500 (p/n: 90559-1) into my iPhone 15 Pro Max and inserted my CAC. Expecting nothing to happen, I was surprised to see it happily blinking as though my phone and Smart Card could finally be friends.

Having previously used a Bluetooth reader, I used the Smart Card Utility app from twocanoes (Smart Card Utility for iPhone and iPad) to verify the reader was recognized--and it was! The final test was downloading the DoD Root Certs and logging into OWA, which also worked!

The practicality of plugging a Smart Card and Reader into your phone is debatable, but it's a convenient backup option.
 
Best news I read all day!! This was my biggest hope when upgrading this year. Now we just need native support. Would be great to not need a 3d party utility like on our Mac’s.

It didn’t dawn on me to try without the middleware; they’ve successfully conditioned me! However, I deleted the Smart Card Utility app, restarted my phone, and opened up a private browsing tab to see what would happen.

It appears to work natively! You’ll still need the DoD root certificates profile (which Twocanoes has on their website for free).

Additionally, the same is true for my M1 iPad Pro—which has not been the case since I bought it. It would seem like iXXOS 17 enabled some new port behaviors.
