I'm trying to work out what I need to connect my 15" Macbook Pro (late 2011) to my HDMI television.

I'm told I need 1) a Mini DisplayPort to HDMI adapter and 2) an HDMI cable. First, is this the case? Secondly, does a combination of the two exist in one cable?

Yes, that's what you need. AFAIK, there's no combo, just purchase the adapter and as long an HDMI cable as you need.
 
Both options do exist.

The mDP-HDMI adapter is more common than the cable with different ends, though.

I use an adapter (from Belkin, if I'm not mistaken) with a regular HDMI cable myself on a 2011 MBP and it works well.
