Custom icons randomly shrinking??

Not really mac or bootcamp related, but i download and use custom icons for my games in windows. I download a 512x512 .PNG off google and then convert to an 256x256 .ICO file. i set the icon in properties and all is well. But randomly the icons will shrink (see pic). why is this and how do i stop it??
