Display when incoming/during call?

I know this is stupid, but just a little thing thats bothering me. How would one get the iPhone to look like it does in the first photo below? The difference is how the contact image is displayed and the text at the top.

Add the image in OSX address book and not on the Iphone.

Yeah, if the picture is too small instead of stretching the image, it puts it in the small box. Elegant!
