Do apps transfer?

If you pay for an download an app, (such as the seemingly great MLB.COM app), on an iphone, then exchange your phone for a new one, what happens to the app? Does it transfer? Does it go along with your phone number, (AT&T account), or is it physically lost to the phone you turn in for an exchange?
