iPhone Do Not Disturb Keeps Turning Itself On (iPhone 7 Plus)

You have it completely disabled, including scheduled section as well as Bedtime section? Do you have any other devices connected?

Now it just randomly comes on throughout the day. I’ve missed calls and texts, not realizing the DND ghost turned it on. I hope it’s just a bug and Apple will push out a fix.
As far as I know everything was disabled. I think the only way to fix this bug is to configure a specific period for DND such as 3:00 to 3:02 AM, then it will be enabled but not bother you when you don't expect. And since it will be ON in a moment you will not use the phone (and for just 2 minutes) it will render the feature useless.
I believe you can schedule it to be on and off at the same time and that will basically turn it off at that time if it was on for some reason.

I am having the same issue with my iPhone 7 plus. For me, there is nothing to indicate that DND is actually switched on (no icon), but the behavior is most def DND behavior and I can't disengage it.

all - was having same problem. New phone w ios12. For whatever reason, dnd was set to ‘on’ in the alarm app when I had the ‘bedtime alarm’ on, which I use every night. Turned dnd toggle to off in the alarm app, fixed the issue.

Apparently, in the Clock app, on the Bedtime tab, in Options there's an option "Do Not Disturb During Bedtime." (https://tidbits.com/2018/09/19/inside-ios-12-do-not-disturb-learns-to-turn-itself-off/) This setting isn't reflected in the DND settings for some reason. It's probably your culprit.

I have this same problem. Despite having a schedule for 11pm to 6am, DND is on outside of those hours. Like right now it is 6:37pm and it is on when it shouldn't be. Bedtime is off in Settings->DND, and "DND During Bedtime" is off in Clock/Bedtime app.
OK, here's what I found out:
Do not Disturb while driving on = enable manually (in other words = disabled). All the rest from what I saw it's off.
Bedtime alarm is on (I guess that's what it is).

I think I have fixed this problem with my iPhone I went to Clock. Bedtime. Options. Turned off the dnd at bedtime. It seems to have fixed the really random dnd that was happening throughout the day.
