Do you have to give the apple store your name and information when purchasing?

I purchased my very first iPad at the apple store and they asked for your name and number. Does one have to give that information ?

Well, assuming the iPad is for you...it won't be much use without an Apple ID. In which case you will have to give Apple a lot more than just your name and number.
