Photos on a Watch are synced from the phone and can't be created on the Watch currently.

Is CSAM functionality coming to WatchOS only to support the Communication Safety feature in Messages?
(keep a user from from viewing sensitive messages if parental controls are enabled)

Or does it imply a camera is coming to Watch? Thoughts?

I don’t think there’s a camera coming to the Apple Watch in near future, and I don’t think that if a camera is added one day, it will be at all related to CSAM new features. The Apple Watch will likely only get the communication safety feature of iMessages, as iCloud Photos will be processed locally on iPhone and not AW (as far as I know). I would be surprised if the hashes matching could be processed locally on the Apple Watch

I agree on the processor load, all would be done on the iPhone and we don't know the size of the hash table but it's probably big enough to not fit easily on the Watch.

CSAM + a camera would imply a cellular Watch would either have to be able to process locally created photos or that uploading to iCloud Photos couldn't happen if away from the iPhone even if on cellular.

I just figured if we are getting a form factor upgrade, it's almost now or never for a camera.

I really don't see how they could even implement a camera that's useful on the watch unless you just want to take selfie's...

Camera on the iPhone is a battery and RAM hog, esp when you use advanced features like portrait, night mode and such…
No camera on watch please Apple. And if it did, might not get it
