-I've always known the standy time included the usage/screen on time (dumb but whatever)
-But that said, the usage time is just crap:

50% battery left on my iPhone 8 and 5 hours usage is just not right. The screen hasn't been on anywhere near 5 hours, and the phone really hasn't been doing any screen off background activity either other than a half hour of music. Background app refresh is off too.

It seems that you’ve been playing music. That counts towards usage time even when the screen is off.
 
Background activity counts every minute.

Music does too! Listening to music for 1 hour will add 1 hr of usage to it.

If you click the clock it will tell you how much of each of those percentages is on-screen versus background, down to the minute.

The usage versus standby was never a “screen on” versus “screen off” statistic.

My biggest one is google allo. 25 minutes on screen and a whopping 1.5 hours background. That ALL gets tallied towards usage.
 
Various system sync and other activities are also part of usage even if the screen might be off and the device is not in use. Often those might not show up as anything in particular as far a list of items for battery usage goes.

Keep in mind the clock shows app activity for the last 24 hours from your current time. That’s why it won’t match up with your usage stats, it’s not a “activity during this charge” indicator.

That's a good point and I think that's something that's easy to forget.

See now this morning, I have 24 minutes of usage and 1 hr 45 minutes of standby and I'm sitting at 100% battery. Screen has been on for maybe 3 minutes total. I realize it isn't strictly screen on time, but last night before I put it on the charger I killed all apps and did that home button RAM reset and restarted the phone just to make sure everything was off/killed, so I have no idea what has been going on in the background for 24 minutes already in the last couple hours. Oh well I guess.
