Had a pretty noticeable dust particle within the outer camera lens when I unboxed my XS. I always feel a little uneasy when returning stuff but I knew this would drive me crazy. Asked for a new phone saying that if the particle moves over the center of one of the lenses, it may distort the photos and affect the performance of the camera. They exchanged it without question.

What I’m saying is, it could’ve always been there and eventually made its way to more noticeable area. Don’t know how a dust particle of such noticeable size can enter from the outside.
 
Yep, you’ll be covered under AppleCare+ and they should exchange without question. Just make sure you back up your data and wipe before you go
 
Does Apple Care+ take care of this. It is very noticeable and I don't know how it happened.

If it’s dust (Which it likely is), Apple can actually disassemble the camera assembly module, and clean behind the lens cover and reassemble the phone. [They have been trained to actually do that now since over a year ago.] If it’s a scratch inside the lens cover, which seems to be rare, then they’ll just replace the phone. But it’s likely dust.
