I think you have to go to Music > iTunes > and then add them to either "Home Videos" or "Movies". They should then show up in iTunes Library

I have come across a related problem - using the Device/AppleTV preset of Handbrake has started producing mv4 files that macOS cannot read. Even clicking on the file in Finder results in the forever spinning beachball. Not surprisingly they will not import into iTunes. Using a general
Preset like HQ 1080... seems to fix the problem.
It seems that Handbrake has a bug or macOS has (been?)changed to reject some types fof Handbrake-generated video!
Update: A quick check has revealed that many videos that I have created in the last few months no longer play in macOS 10.14.2 (ie Quickview doesn't work ) but they worked fine when I first created them. Older ones with mV4 or MP4 suffixes still preview in Finder so it seems to be related to the version of Handbrake and Mojave.

This has been going on for the past 2 or 3 versions of MacOS. Specifically, I think it is due to changes made in QuickTime. But you don’t need to re-rip everything. VLC, Plex or Infuse will play all these problem files flawlessly.

Yes - Quicktime 7 works as well but not iTunes or the Computer app of Apple TV which are essential for my needs.
Still trying to find a Handbrake preset other than General/Fast 1080p that works.

See if MDRP can do what you need. There is a 5 instance free trial. It should be what will do the trick. Great program I have used to archive all my DVDs.

Make sure that your encoding settings used are actually supported by iTunes. It is quite limited, and MP4 is simply a container and doesn't tell you anything as such about the codecs used.
