Is it possible to save the image of a QR-code on an watch and have it easily accessible to display full screen?

I need it for a specific use case: When I shop groceries I can get a discount through a loyalty program, but that only works when I pay with a specific card. It doesn't communicate through Apple Pay when using the watch. A workaround is scanning a QR-code at a reader next to the register before using Apple Pay.

Is there a smart way of doing this?

You could put the image in the Photos app on apple watch (by syncing photos from your iphone to the watch)

I store all my reward cards, gym access, or any others items needing a bar or QR code scanned in the Apple Wallet. While some items have native access to the Wallet I use this app to create the ones that don’t: https://apps.apple.com/us/app/pass2u-wallet-cards-coupons/id1142473931

Having these in Apple Wallet make it very easy to access and scan from the Watch.…I hardly ever use the iPhone‘s wallet for that purpose.
