Just to be clear, are you trying to get rid of the icons, the UI changes, or both? Also, were all the changes caused by the one application, or did you install multiple UI mods?
I nuked all the brushed metal elements long ago (with UNO, but not before several weeks of checking it out to make sure it was safe and undoable). Maybe if you shared what you can remember about where you found the mod (if anything), we might be able to use our collective seek and destroy abilities to figure out the name.

I do like the way it looks I think, it's just kind of annoying to feel like I can't get it back to normal.
But I'm trying to get more at piece with the fact that it would be really hard to find what I installed probably... and who knows, maybe just someday I'll come across it.... so it looks pretty good and I can just live with it for an undistinguished amount of time.

I know the name is one word, and it was a compressed file that when decompressed was a folder w/ 3-5 items in it. Also, it had it's own webpage I think....
if you remember the day you downloaded it, check your browser's history to see if any sites jog your memory?
