So...

Yesterday my iPhone XR slipped out of my pocket as I was getting out of my car causing several impacts on the glass and some minor (in my opinion) scratching to the chassis.

I am in the iPhone Upgrade Program and have AppleCare+. I called Apple Support to start an Express Replacement repair and was connected with someone in Australia. He was very helpful explaining the whole process and said the fee would be $99. I was a little surprised by this because at most I was expecting a $29 screen repair fee.

Has anyone done an iPhone express replacement recently? Is the $99 fee correct for any Express Replacement service?

Yeah, if you do an express replacement online you pay the full repair fee as they replace the entire phone. If you take it to store they just replace the screen so you pay the screen repair fee!
 
Chassis damage is considered "other damage" and is $99.

What is the threshold though for "other damage?" Any knick at all? For instance, if I went to turn an iPhone in as part of IUP, with knicks on it would Apple reject it?

As far as I know, small nicks and scratches on the chassis are ok. Cracks on the front or rear glass will need to be repaired.
