It's like Skype since it's actually VoIP. Not sure how that transition works though as you'll be talking to the person on the phone before initiating FaceTime.

It won't use your minutes.

And you do not need to be in a current call to initiate a facetime chat. It actually says this on Apple's site.

What worries me would be that, if I know someone with an international number, I would have to initiate the international call before I can initiate FaceTime.

That means AT&T still got me for 1 international minute.

Since AT&T supports both data and voice at the same time. Why not use the voice channel to handle all the audio, and use the data channel to transmit the video. Therefore, it is not true VOIP, but a hybrid version of it.
