Finder keeps asking if I want to open an app

For some reason, my system is constantly asking if I want to open an app (Notational Velocity), no matter how many times I tell it to. This is the only app it constantly asks about. I use it a couple times a week, and have been since July 2, 2011(!), so I'm pretty sure that, yes, I want to open it.

I have tried repairing permissions, to no avail.

Any ideas? Should I delete and redownload?
