Fluctuating Call Volume/Can't Adjust Volume

Using earphones:

I am finding that the call volume fluctuates for some reason, and on the lock screen I cannot adjust the volume with the inline remote nor the side (physical) volume keys.

I've noticed the volume increases when I type (key type sounds on).

Anyone else have the same issue.

I also have this issue, it seems the volume won't increase or decrease on the Lock Screen while taking a phone call.

I have this issue too using bluetooth handsfree, only way to fix it is hang up and call back and it brings volume control back for a random amount of time before it goes again.
