I am going into the apple store tomorrow to get my iPhone 3GS checked out because it seems to be convinced that headphones are always connected to it...

I am on 4.2.1 (JB with greenpois0n). Should I update to the latest (and drop my JB?) How do i make a backup of my phone first so I can go back to it?

Update to 4.3.5. make sure you save SHSH blobs incase you're stuck with that iPhone, and I'm not sure since I've never used it but I hear TinyUmbrella can save most of your stuff.

No, you really don't have to. Since your issue is most likely a software issue you NEED to restore/update it to the latest software to make sure it isn't software. THEN you'll need to go to the Apple store to see if they can replace it.

More likely than not, they won't help you if your iPhone is jailbroken.
 
so how do i completely remove all traces of my JB then???
