Graphic embedded over my prints

Hi,

I have been battling with this issue for years and I am yet to find a resolution.
When I print from mac OS I get this graphic embedded on top of the document I am trying to print. It doesn't happen every time and only with graphic text / pdf documents.

Any ideas? Yes, I have reinstalled my print drivers.
