This is an old thread. No need for 3rd party app now because you can set heart rate alert on Apple built-in Workout in current watchOS.
Hi…;-). Sorry but this is not correct. Apple doesn’t have any app that lets you set a heart rate alarm with sound or haptik. Apple’s Health app has a heart rate alarm that notifies you but there is no sound or haptik. Apple’s training app doesn’t have this functionality either. I have put in a lot of effort to identify an app (Apple or other) that lets you do that and the only one I have found is the app Heart Graph.

You can set your desired HR zone when you set up your workout and you'll get a haptic (not sure about sound, my AW is on silent all the time) alarm when you get below or above that zone. It's all built right in.

How do you do this? I can't find any info about this.
 
On your watch: Workout app, you see a list of workout types. Click the three dots on the right, you see a list of all workouts of that type. Create a new one or choose one, click the pencil button on the right, then the row with the bell ("alerts" in my language), choose the heart rate one, choose or enter the desired zone.
