My mom isn’t the most tech inclined.

She keeps activating voiceover on her Apple TV/ and eventually will just turn off the tv because she gets annoyed she cannot get it off, honestly with how loud it is I don’t blame her.

In the settings I don’t see any easy option to either disable it permanently, turn it down so low you can barely hear it when it is on, or change the activation method to something more obscure.

Is any of this possible?

Either she’s activating an accessibility shortcut, or she’s inadvertently activating VoiceOver rotor controls.

For shortcuts, navigate to Setting > Accessibility Shortcuts; if VoiceOver is there remove it.

My bet is the latter - “people of a certain age” (including that old guy in my mirror) find touch controls to be fiddly and to do unintended things. If she happens to have the old black remote, buy her a new one. And if/once she has the new one, Settings > Remotes and Devices > Clickpad > Click Only.

That was exactly what she is doing, she clicks the “menu” button fast when she gets frustrated something isn’t working and it was activating voiceover.

I somehow completely missed the “shortcut” setting you pointed out (several times) and obviously wasnt googling correctly, but using it I was able to completely disable it!

Thank you -so- much for taking the time to reply, I am hopeful this will solve a lot of issues.
