Unfortunately, the only way appears to be completely restoring (erase) the device and resyncing it as a new phone.

Get whoever the bill payer is to call your provider and ask them to unblock your simcard.

EDIT: Oh right, wrong block. Hmm, it takes a prohibitively long time to get to 10 wrong attempts. Fishy....

try the last 4 numbers off your social security number sometimes that is what the code is

unfortunately if you set the code you will have to restore your phone.
