Hi, I will list what I have first:
Apple TV 4K
Samsung UN65KS8000 Tv.
Nad M15HD Pre amp.
Nad M25 Amp.
The Nad can decode all audio formats except Atmos (waiting for module 4K which will support that)
Right now I have Apple TV to Samsung through hdmi 4.2.2 and since the Apple TV does not have a second hdmi out like my 4K Blu-ray player, I’m sending optical signal from Samsung TV to the Nad Pre Amp optical input.
I have the TV set to bitstream since PCM would be uncompressed 2 channels only.
Do iTunes movies have all audio tracks in them? Since the wat I’m doing it, I think, I can only get DD and DTS right?
Dolby TruHD would only work as uncompressed and it needs hdmi to work?

From appleTV you can only get DD5.1 (internally re-encoded) and PCM multichannel, up to 7.1 (internally decoded DD+7.1).
This is native tvOS.
You may be able to find an app that would support audio passhrough. I have no experience with them.
