It just may be a coincident but ever since updating to 10.13.4, I have been receiving a Kernel Panic.

It mostly happens when I place it to sleep.

I ran Tech Tool and Disk Warrior on items I could and all tested okay. I do have one back-up hard drive with bad sectors.

I did the normal resets without a change.

It is a 2010 MacPro with 2-3.46 GHz/64GB RAM, flashed AMD Radeon 7950, Sandisk Ultra II 960GB, 4 hard drives

Maybe a USB device. Pull all USB stuff for test sake.

Also, may be you should download the 10.13.4 full installer to update your cMP's firmware, just in case that fix some strange APFS issue.

I suspect a failing hard drive...

Okay...did download and tried the combo updater. It did not ask me to update my firmware...I did do that when first installed 10.13.

Here are my reports on the one hard drive...it is for my Time Machine. I do have another drive ordered.

I went ahead and pulled the drive out while waiting for the replacement so will see what happens in the meantime.

I have some USB items unplugged as well. I do have a USB Bluetooth dongle to avoid the 2010 cMP issue...never had any issues before though with it.

The Combo updater is NOT a reinstall of your macOS system. It can rebuild what it is capable of providing, and can fix a variety of errors, tuning up your system in some ways, but it is not the same as a system reinstall.
The OTHER test you can do is to remove the time machine backup drive, and try using your system for a day or two.
Do you get other kernel crashes, even with that time machine drive out of the system?

With the bad hard drive out...still received the error. I am noticing the apfs error.

My new hard drive to replace my failed Time Machine drive came today.

While installing it, I did unseat and resinstall the cards, the Sandisk too, to make sure all is okay...also cleaned the thing out (pet hair, etc.!).

Time Machine is running its first back up.

Prior to updating to 10.13.4, I did have issues with it crashing from wake. This issue just started after updating to 10.13.4.

The Sandisk SSD is your system boot drive?
How is that installed in your MacPro? Which slot?

Yes, the Sandisk is my boot drive.

    It is in slot 3.

It has been there for well over a year though.

    Do this and report back.
Download the FULL INSTALLER from the App Store, NOT the Combo updater, and update your cMP firmware.
Don’t worry, it won’t force you to reinstall the OS. Just update the firmware and quit the installer when done.

Okay...tried and it only wants to install the OS...any way to find or force a firmware update?

Yes, I downloaded from the store...just finished running it.

    My current firmware:
    Boot ROM Version: MP51.0084.B00

    I have a flashed AMD Radeon 7950

I do have a drive with 10.12 on it and will try that and see what happens..

Okay...I finally got the Firmware updated. I had to use a disk with 10.12 on it in order to download the needed version with the firmware.

    I am now at:
    Boot ROM Version: MP51.0085.B00

I noticed my panics seem to be apfs related...does this firmware address that?

is your drive APFS or HFS+?

likely simple fix: clone to HFS+ drive and replace in system (carbon copy cloner)

My start-up SSD with the panics is APFS. If this continues, can the SSD be reformated to HFS+?

I do have a clone of the drive but it is on a standard drive (slower and HFS+).

Yes, you can of course re-format the SSD back to HFS+, just make sure you format the "drive" not the "partition" in Disk Utility.

If you have a clone already. Then boot to the clone, format the SSD, and clone it back. The whole process should be very straight forward.

I was starting to think my problem was fixed after updating the firmware...unfortunately, woke up with the kernel panic.

It seems like it only happens when I have it sleeping.

Is it worth trying to reformat the SSD?

If you can, clone drive APFS system drive to HFS+ formatted drive and replace. Carbon Copy Cloner can do this really easily. Should be pretty quick and easy to figure out if that is the cause or if it's something else.

Notice "Bose USB Audio" in your list. What port is this connected to? A built-in port, or a PCIe port? I've noticed some issues with legacy USB/FW audio devices recently. If there are firmware updates for your Bose device, you may want to take a look at updating. May be completely unrelated, but disconnecting for a few days could help troubleshoot or eliminate.

You seem to have two separate UPS attached. (ES 550, and RS 1500) Is that correct?
Maybe I am interpreting that wrong.

Thanks...do use CCC and will try the reformat.

I have an older Bose Companion 5 connected to the factory USB port in the back...will look for updates as I did purchase it new back in 2008.

Yes, two UPS's connected...I use Slim Battery Monitor to check on them...I have one UPS for my Airport and router and the other for my computer.
