How do I remove an option from text forwarding?

I was wondering if anyone knew how to remove one of the options listed under text forwarding. I used it to forward to my old iPad, but t a new air 2. How can I remove this old option? I wiped the iPad because I planned to sell it, but it's still listed.

Exactly the same for me, tried everything to no avail....

Can't you just toggle the device name OFF in Settings > Messages > Text Messages Forwarding on the iPhone?
