iOS How do you complete form W-8BEN (UK Citizen)

Hi there,

I am trying to sign up to iTunes Connect in order to potentially sell iBooks for money.

I have to fill in a form (W-8BEN) for tax purposes, but I have no clue what to do with it.

Can any one from the UK tell me how you are supposed to fill out the form? I am a UK (British Citizen).
