I also swap and change and am never satisfied with the way it looks and how to organise it, so how does everyone do it?

Maybe post photos and gives us some ideas?
 
I tend to just follow the layout that's always been on my iPhones, no logic or reasoning just place the apps I always use in the same location on the first screen, and then the rest wherever thereafter. I tend not to have any folders on the first screen either, just the apps, then folders on screen 2 etc...

I hate covering up wallpaper with icons. What's the point?

I've been jailbroken for almost a year now (on iOS 6) so I have five icons on my homescreen and dock which allows me to center them (one icon is always dead center). In the dock, that's my most used items all in one folder.

Here's my first five homescreens (I have several more, all grouped by functionality, theme or purpose).

The first screen is clear just because I like one screen with no icons (except for the dock). My iPhone is jailbroken so you won't see much in the way of how things are presented if I were stock.

for me:
top row: calendar, photos, weather, camera
second row: maps, mail, app store, settings
the other rows don't really matter,
the dock is always music, phone, safari and messages.

i don't get why a lot of people keep the message app in the top left when for most it is the most used icon, yet they keep it the furthest away. I also try to keep everything on the first page so i have a row or two of folders.
 
Most used apps on first page (only go 4 rows, leaving the fifth open), then folders on second page.

In the dock I have phone, messages, mail, safari.

I've thought about mixing it up and doing some different things, but I'm used to the way it is now and really spend little time staring at my home screen.

Main dock= phone, messages, mailbox, work email

First page= main and most used apps. No folders!

Second page= social related and news/blog

Third page = misc apps and folders

Forth page= games apps
 
Years ago, I moved the Phone app out of the dock, which was weird given that it's a phone.

I like to put the most used apps on the dock, followed by the bottom right corner 2x2 area as it's where my thumb naturally reaches.

It can sure be tough making changes after being used to a setup for so long. Especially now that so many icons look the same to me (photos, gamecenter, google drive, remote, safari).

I guess we have different definitions of clean then. When I say clean, I mean I don't have a bazilion app icons in my dock or on my springboard. Yes, there is info in the statusbar and I use DashboardX to put two NC widgets on my homescreens and then a set of 5 app icons under that.

But the space between is empty and you can see my wallpaper.

If you're meaning all those gray bars in the background, that's Livepapers, a tweak that allows animation on my homescreen(s) and lockscreen.

Compared to the average stock iPhone user who has every space filed with an app icon, is my layout not clean?
 
I have my iPhone and iPod touch generally basic for the first pages and on my iPad I have lots of productivity apps like Pages and Skydrive because I use them a lot.

A lot of folders with most used on first page.....I keep all iDevices set up the same. My wife calls hers organized but she can't explain the organization. I think it is more memorization than organization.
