Is it the same way as Unix does, or in some file like .pst with all the inherent problems?

No. Stores mail boxes in packages with .mbox .imapmbox etc extensions, and within those stores the actual mails in .emlx files which just store all the typical mail header info plus the actual contents of the email including all css stylesheets etc... it does now however store the images from the mails

You can find it all in /user/library/mail/
