I'm running Mojave 10.14.6 on a MacMini 2018.

How to customize the Mojave context menu to be able
to hi-light a word, in any app, right-click it and in the
context menu an option to "Lookup in Dictionary" appears?

Selecting that context menu option would then open
Dictionary, lookup the selected word and display the result.

It is here in the Keyboard pane of System Preferences.

Seems to work!

Hopefully this applies all over.

I set Alt+D as the shortcut.

However, an additional tweak: how to be able to hi-light a word then
right-click it and have "Look up ..." (or a similar phrase) appear in the
context menu?

I do not get "Look Up "(word") appearing in my context menu.

Do you have a tweak to make this appear?

Another approach is to set the app Dictionary as a login item so it launches when you start up your computer. To find the definition of a word put your cursor over the word and press Command+Control+D

I started Dictionary.

I hovered over a word and pressed Command+Control+D : nothing happened.

Then I selected a word and pressed Command+Control+D : a small version
of Dictionary popped up:

Not only is this not what I'm looking for (I want the full Dictionary app
to appear) but it does not work as you described it should i.e., merely
hovering over a word and pressing Command+Control+D, does not
do a thing.

Q: How to get your suggestion working in my Mac?

A reply to my post shows that this is a standard/normal
context menu with the "Look Up" option appearing:
