My watch bugs me every time I get in my car. It gives me this notification assuming I’m going home or not. And whenever I’m home and get in my car it assumes I want to drive to work.
How can I turn this notification off forever?
With that being by said, I enjoy the maps app and getting alerted when I’m using the maps app and need to turn left or right or merge or whatever.

not seeing thos in ios 14.3
It's there...System Services isn't in the alphabetical list, it's all the way at the bottom. Then when you go into System Services, Significant Locations is at the bottom of the first list. You might also want to turn off Location Based Alerts and Location Based Suggestions in that same menu...or experiment with them to see which one controls the thing you want to turn off.

rather than turning off what you suggested in settings on the iPhone, I did so within the Watch app and then within Privacy turned off Map suggestions and seems to have done the trick. Specifically turned off Driving with CarPlay.
