OSX never keeps battery at 100% all the time, it lets it drop to 96 and charges it back to 100 from time to time when it notices that computer is connected to AC for extended periods of time.OP:

Are you someone who "uses it unplugged" and plugs in the charger only when needed?
If you're of the former, remember that the battery "needs exercise" (for lack of a better term).
At least twice a week, take it OFF the charger and run it down to, say, 40-30%, then re-connect the charger.
Post a note on your kitchen cabinet: "Mondays and Thursdays are battery days".
