What kind of network do you want to create?

Do you use the AE to connect to the internet via PPPoE?
If so you need the number your ISP provided to you for dialing up.

I have an AE and it was fairly easy to setup.

I connect to the internet via PPPoE, and created a wireless network with the built-in tool for ""Create a wireless network".

I didn't had to use the fields you made a screenshot of, but with it may vary, if you have other plans.

The Manual does not help at all it just says "Open the AirPort Setup Assistant, located in Applications/Utilities on a Macintosh, or
the AirPort Express Assistant, located in Start > All Programs > AirPort on a computer using Windows XP. Follow the onscreen instructions to create a new network."
 
So, again what kind of network are you setting up?
Just a basic WiFi Network or something alittle different?

Id restart AirPort Utility and when the AirPort Express shows up you click continue and allow it to configure.

If this is your sole WiFi router, make sure it is plugged into your Cable/DSL Modem.

If it is not your only WiFi Router and want to extend or have it join your Current WIFI Network; then simply choose Connect to existing network
