Can anyone tell me how to use AHT on a 5,1 Mac Pro? It won't boot with it internally or with a USB stick. I probably set it up wrong but I don't know. Any way that would work would be great!

Follow these instructions:

You need an Apple OEM GPU (HD 2600XT, 8800GT, Quadro FX 5600, GT120, HD 4870/5770/5870) to run AHT/ASD. AHT/ASD can be used with PC flashed GPUs, but will fail mid test when the GPU is checked. Non flashed PC GPUs won't work at all.

MP4,1 with MP5,1 firmware uses MP5,1 AHT/ASD.

What is interesting I tried recently AHT with GT-640 ( single slot flashed) and the test did not fail.

I'm also in need of using the bootable AHT USB stick. I would be using the Mac Pro 5,1 Mac-F221BEC8 file.
But I feel the installation instructions are bunched together and a little confusing to understand. Can anyone space out the process to make it easier?
