I Need The Amazon Rebate Form!!!!

I ordered jaguar earlier this week from amazon to get the rebate but did not save the rebate form. And now I can't find a link to the form at Amazon... Does anyone here have a copy of the form that you could email me..... If so PLLLEEEAAASSEEEE do so. So that i too may get the discount that amazon has now gone back on on. Thank you!
