Here is an idea:
Manufacture an IR emitting device that the consumer plugs in and sets on a table in the room. The device connects wirelessly or with Cat5 to the consumer’s in house network. The app then tells the emitter what IR signal to blast and when to start, stop, etc. the DVR. The app designer could then write an additional app to make the iPhone into a fully programmable IR remote control for the entire entertainment center.
 
I have U-verse, and while remote programming is a great feature, a few simple improvements (which could be added to i.TV) would help imho. In my case, I prefer to record the HD version of my favorite shows, but since they are generally the channel number plus 1000, they don't show up without scrolling through several pages of offerings. A simple option to search HD first would be great.

This won't address your entire problem, but are you aware that if you press and hold on the right side of the channel listing a scroll bar appears and you can move quickly anywhere in the list (just as you can with your ipod controls)?

Also, from the channel listing screen, touch "sort" and then check High Definition as a required attribute. I think that will then display only content or channels that are presented in high definition. I haven't tested that completely yet.
 
If you want a UK tv guide, I have found the one called (imaginatively) tvguide on the UK app store to be very good - lets you customise channels and review in landscape mode.

Don't think it allows you to remote control anything but as a tv guide its quite neat.
 
I've been playing with this app for a few days now, and other than the fact that I'd like to see more than a 30 minute window, it is by far the best Tv Guide available on the iPhone for me.

Directv user, HD DVR's.
