After having to reboot and set my time and date again (never had that before) my iCal Alarm pops ups every now and then but with an empty calender item.

There is no way to cancel this or a button to press. It seems to me the process takes a while with a lot of computing (and hard disk work) and then the box disappears. After a while the hole thing starts again.

Never had any problems with my iCal of syncing with my iPhone.

Please help how to fix this...

What OS X version? What iOS version? Do you use iCloud? Do sync calendars with any other services such as google?

I have not had problems with .mac, mobileme or icloud sync so I'm not sure I understand your issue. I did experience some issues trying to sync to a Blackberry or trying to sync to google but I would think those are pretty much in the past now.

One thing to do is turn off ical alerts (if you don't need them). More here...

Sorry for lacking this information: I am using Mac OS X 10.5.8 not syncing via iCloud only with cable to my IPhone 3GS (IOS 5), no other devices in use like google.

It is really straight forward at my place, but after the reboot I had to re-set my date and time and the date. Every little now and then the iCal pop up comes around but with not display item and simultaneously my hard-drive starts spinning like mad.

Sorry I can't help you with Leopard. I only have ML, Lion and Leopard (on an old G4 that's currently switched off right now). I do not know of a "sync" solution for pre-Lion users since iCloud requires Lion. Do you have Spotlight set to search your calendar? You might switch that off.
