My Macs (MBP2016 and Mini21 OS11 and now 12) keep adding old exchange accounts. The notification would say "Exchange account added" and the email address would be displayed.

The thing is: The accounts don't exist anymore and there is no reference to them in Keychain. If I delete the accounts 'from all devices' then a few hours later or the next day, they would just pop up again.

Where does the OS get it's info from and how can I get rid of it?

Did you have any luck with this? I'm having the same issue and have tried a bunch of steps to debug this myself. I called Apple Support and they gave me a few options to try but ultimately settled on 'turn off iCloud Keychain' which doesn't seem to be a proper fix.
 
I narrowed it down to an iPad that I suspected was the one that kept feeding these old accounts to the keychain on iCloud and then onto the Macs. Unfortunately there is no keychain viewer on iPad (that I'm aware of) and no keychain viewer on iCloud to check.
In the keychain's on my macs I had deleted the old info so they could not be the cause.

I completely wiped and restored the suspected iPad and haven't logged into iCloud on it since. Also switched keychain sync off and back on, on the Macs. Problem gone.
 
I had the same issue, and I resolved it in this way:
Last year I added a outlook account via exchange; I removed the account (from internet Account and from keychain, but after restart the laptop or reinstall the system the inactive account was back. I add a different outlook account via exchange, them I delete from keychain 5 or 6 entres named SOSDataSource-ak, and the file with the name exchange. Close, restart.

Problem resolved.
