Here's the PSD for you people...(including the Myriad Pro Semi Bold OTF font)

You must install the font for it to look right.

The file is simplified from the OP's instructions. 1 font layer, with gradient overlay, inner shadow, and a 1px white drop shadow, both set to 90 degrees, to better mimic the lighting of the tag.
