Hello! I'm looking for mac software that will allow me to create a checklist for a patient. If the patient checks on a certain box (say "heart attack") then, when they complete the first page, an appropriate second checklist will pop up (when? how was it treated? current chest pain? etc.). If they check multiple boxes then appropriate second and third checklists will pop up in order (i.e. they check heart attack and diabetes, then the heart attack questions come up followed by diabetes questions when they hit next). This would be a web based entity. I hope this makes sense. Thanks!
 
How will the patients access this? From anywhere (e.g., their homes)? From workstations setup in In your office?

And how do you need to be able to view the info they've entered? E.g., are you looking for aggregated results or are you going to print it out or file it somewhere?

Also, I'm assuming you have no experience or interest in web development, is that correct?


Ideally you'd find someone that offers a medical form creation and hosting service for low $/month. Like a YouTube for medical forms. Unfortunately, I don't know of one. If you can't find something like that, I know there are services that allow you to create and host general questionnaires and surveys. Perhaps one of them will work for you. Google away if you can't get specific recommendations. You'll have to watch out for stuff specific to the medical field like HIPA requirements, though -- a general survey service may or may not work.

E.g., look at the "Publish data to the web" section. Filemaker does a decent job at allowing technical-minded non-developers to create straightforward UI-driven data collection apps.
 
Thanks for the help. All the issues you mention will be vital at a later stage. Right now I want to set up the "page" with the information I want then discuss it with a developer.
