Late-2015 27" iMac 17,1, Intel i5-6600 @ 3.30 GHz, 8 GB RAM.

(Edit: I know that this is a painful read, but the tech-heads that like this stuff need this sort of detail 😉)

The internal HDD is physically installed and is functioning normally. It still contains the original macOS installation and user data. Nothing has been erased or reformatted.

The objective is not data recovery. We simply want to wipe the existing installation and perform a fresh macOS Monterey installation.

The original owner is my mother, the only owner of the machine. She is no longer able to operate it, so we do not have a reliable way to authenticate to the existing account. None of the passwords available to us have worked, so I am currently locked out. This is not being presented as evidence of a password or security failure. It is simply why we are working from Recovery rather than the existing installation.

The machine's firmware is well past the known iMac17,1 170.0.0.0.0 firmware problem and has been updating normally, so that historical issue has already been ruled out.

We are currently booted into macOS Recovery, not normal macOS. Recovery itself is functioning sufficiently to provide Terminal and its own APFS/Base System environment.

The actual problem:

  • Recovery is not exposing the physical storage layer.
  • diskutil list shows the Recovery-related devices. A disk3 object appears, but it is not backed by a physical store.
  • diskutil apfs list reports:
    • Container ERROR -69810
    • No Physical Stores
    • No Volumes
  • diskutil info disk3 reports:
    • Whole: No
    • Disk Size: 0 B
Most importantly:
  • diskutil list physical
  • returns no physical disks at all.
Therefore this is not simply “the HDD is blank,” nor an APFS formatting problem. The existing HDD has an operating system and data on it; Recovery simply isn't exposing the underlying physical device to us.

External storage:
We've also attempted to use USB drives/external storage as alternate boot or installation media, with the same basic result: the Recovery environment does not make the physical external storage available to us.​
That makes “the internal SATA HDD has failed” an insufficient explanation. The apparent inability to access storage extends beyond the internal device.​
And there is another oddity:​
Keyboard input becomes unavailable at particular points in Recovery where input is required, including attempting to enter a Wi-Fi password. We can use Ethernet/LAN, but keyboard behavior in Recovery has been abnormal only in recovery mode. It is a wired keyboard.​
We are specifically interested in whether the storage-access behavior and keyboard/input behavior could have a common cause, rather than assuming they are unrelated failures.​
system_profiler is unavailable in this Recovery shell (command not found), which appears to be normal for this minimal Recovery environment and is not being treated as evidence of hardware failure.​

Current hypothesis:

We are not assuming a failed HDD.​
The working hypothesis is that something at the EFI/boot/Recovery/security-policy/environmental layer is preventing access to the physical hardware, or that some other mechanism is causing Recovery to operate without exposing the underlying devices.​

The evidence currently is:

Existing internal storage present + physical disks absent from diskutil list physical + external USB storage unavailable + Recovery itself functional + anomalous keyboard/input behavior at relevant Recovery stages.​
We therefore want to identify what gatekeeper or boot-environment condition could produce this behavior.​
We are deliberately investigating that before opening the iMac or replacing hardware.​

Please don't start with these paths:
  • “Just use Recovery.”
  • “Open Disk Utility and erase the drive.”
  • “The HDD is probably dead.”
  • “Replace the HDD.”
  • Generic APFS/reinstall instructions.
  • Generic “reset your password” instructions.
Recovery is precisely what we're investigating. It currently cannot see any physical disk.

We're looking for someone familiar with Intel Mac EFI/Recovery internals and hardware enumeration, who can help identify why the physical layer is unavailable rather than simply assuming the hardware behind that layer has failed.

We currently do not have another Mac. We might be able to scratch up an old iPad from somewhere if need be. We are technical, but not Mac wizards.

If anyone needs more detail, please do not hesitate to ask. This has been a head-scratcher!

*Devil's advocate* Why crack it when something at the hardware level is keeping us from full access to the drive, external drives, tools, and built-in safeguards have been being prevented/blocked? I can throw a new drive in there with no guarantee that we will not be blocked out of that. What is happening is at a far lower level than the HDD. So what is the proposition that you are thinking about? Cracking a case is cracking a case, no biggie (let's face it, an SSD and jamming 32GB into it would make it viable anyway, but why toss time and money on a bricked unit that will not let us in long before it gets to the OS level?).

This is certainly a learning process for me for sure, but logic is sort of dictating a lot of this nuttiness.

This works best with an ethernet connection to the net.

Power down, all the way off.
Press and HOLD these keys:
command+option+R
DO NOT LET GO!

Now, press and RELEASE the power on button.
KEEP HOLDING THOSE KEYS DOWN.

You can let go when:
a. You see the spinning globe (ethernet)
or
b. You are asked for your wifi password.

When the utilities load, do this:
Open disk utility.
IMPORTANT IMPORTANT:
Go to the "view" menu (menu bar) and choose "show all devices".
You can't see the internal drive until you do this.

The internal drive should now be the "top item" in the list on the left.
Click it and click "erase".
I'm not sure what security options that version of DU will offer you.

If it gives you the option to securely erase, choose that.
If it DOES NOT give you the option, but only "erase", then choose THAT.

Does the erase go through OK?

Assuming that it does, you can now quit du and just "pull the plug".
The Mac will have an "empty" internal drive.

OR
You could install a new system and set it up with a simple account (not your mother's name).

It's up to you.

But TRY INTERNET RECOVERY before you do anything else.

I am not exactly sure what you are looking for beyond what is above, but perhaps I did not explain it well. Here is a screenshot of the machine in Internet Restore mode (I can only do LAN as the machine locks out keyboard input when prompted for a WiFi). This is when I had a Macbook in Thunderbolt mode connected as well (reverse connection to see if either direction worked). The iMac also had a thumb drive and a 1TB drive plugged in as well, all of which are blocked in recovery mode (in addition to the main partition on the Internal drive). Errors associated with these symptoms are listed above. If I am missing something, please let me know. That list is in "view all" mode and erase is not an option on any of them, but then again I am not really interested in the 1.14GB that I can see, it is the 1TB or so that I cannot that is the issue.

If you can't use wifi, use ethernet - that's better anyway.

A 2015 iMac should be able to boot to internet recovery.

Internet recovery boots the Mac "from the net" -- NOT from the internal drive recovery partition. An Intel Mac with a failed internal drive should still be able to boot to internet recovery.
If you can't get it to go to internet recovery, then something else may be wrong with it.

You say you have another Mac (Macbook?).
What year was that one made, and what OS is running on it?
This information is important in order to create a bootable USB flashdrive.

You could create a bootable USB flashdrive, and then boot the Mac from that by holding down the option key CONTINUOUSLY immediatley after pressing the power on button.

Once booted from the flashdrive you should be able to use disk utility to erase the internal drive.
 
What operating system is the Mac booting to in recovery mode? If you're not sure you can open up Terminal from one of the menubar options, type the following and hit return:

sw_vers

Yes, it can via Ethernet.

I can do both types of recovery. Yes, exact steps were taken (you can see it from the screen shot). Both internal and Internet yield the same behavior.

I did get another Mac to test with. I figure that they were purchased at the exact same time. I will include a screenshot of the Macbook info. Thunderbolt negotiations seem to work properly, but of course all storage options are prevented from displaying in any recovery mode.

I am willing to do a bootable flash, I have never done one in a Mac environment. I will have to look up how to properly build one.

It looks like this:

I like the way that you think and I have gone down this path since my last post (thank you for your suggestions).

I will have data to back it up to post, but to answer your question; the anomaly of the missing enumeration of storage introduces itself between 11.7.11 and 12.7.4. I can repeat it on the regular. I will get into more details soon as well as what brought me to this conclusion. Yet I have no resolution yet,

Don't leave me friends! I want to post what I have documented! I am just running a few more tests.
 
Alright, I am going to do the best that I can here. Remember, I am am a computer nerd that inherited an iMac (and possibly now the Macbook that helped me get here).

(EDIT: Oh, and that Mist suggestion would have been AWESOME! lol!)

The premise is that I had an iMac on Monterey that my mother (God love her) could not remember the password to. We did not care about data, but I was stuck with a computer that was essentially "bricked" because we could not get to any storage devices externally as well as the internal drive (less a small portion of the boot partitions). I did extensive diags thanks to people like you to find that at a very simple hardware layer I could see devices, but not after that. Even with bootable drives I saw nothing (read above for the nitty gritty).

My working hypothesis became that this was an access/permission/prevention issue rather than a hardware failure. My guiding hypothesis was "PREVENTION, NOT FAILURE" to finding the root cause. Until I was proven wrong (which I have yet to be, but emphasis on "yet"), I stuck with that.

After I was starting to understand how firmware on the Macs come in a different levels (some is embedded, some layers are with the OS, etc.), At the earliest hardware/firmware layers I could see the hardware and devices, but as the OS/recovery environment loaded, storage visibility disappeared.

So when I started seeing Monterey fail to properly enumerate storage on the iMac, even with bootable media, I decided to go backward and try Big Sur 11.7.10. Boom! I could see it all. So I went ahead and formatted the iMac with that. I could now get into it with a new password. So then I went for it with my 12.4.6 bootable and there was still no disk enumeration of any type.

Interestingly, Local Recovery from the working Big Sur installation can see the physical 2 TB HDD and its APFS container normally, whereas Internet Recovery does not enumerate it correctly.

Now iMac was at 11.7.10 and I had a problem going to 12.7.6. So I made a 12.7.4 bootable. Same thing. So I made a 11.7.11 bootable. It all worked. So I upgraded the iMac to 11.7.11. All is working. Any remote attempts to get to Monterey yield no access to accommodate an upgrade.

Here is a funny curious note: Once I loaded 11.7.11, my OS prompted me to upgrade to Monterey. Knowing that my mom is not a hack (she is 87 now), I suspect that she went down this path and maybe that is how she got Monterey on the machine. Now I can do that (and right now it looks like the ONLY way to do that), but I am sitting here wondering what that looks like on the tail end.
