the last two days anytime Mac falls asleep, it can't wake up. mouse and keyboard do nothing, I can see the black screen is lit, but it won't wake up. I get a report saying "mac crashed due to error send report" or something to that effect. Here is the summary of report:

panic(cpu 0 caller 0xffffff800a491b2c): Sleep transition timed out after 180 seconds while calling power state change callbacks. Suspected bundle: com.apple.iokit.IOGraphicsFamily. Thread 0x11c87.

Failure code:: 0x00000002 00000014

It might be a coincidence but I added two Dell external monitors the other day, and did the recent iOS update, just not sure when this issue started. I am away so can't troubleshoot without the other two monitors, but will try unplugging them tonight and seeing if that helps.

Any idea what's happening and how to resolve?

Does this occur when you use Mojave? You did upgrade Catalina right? I had this happen when Catalina first got released, so I got an eGPU box and have been using that since. Just the other day I turned off my eGPU and have both monitors connected to my iMac directly. I am not seeing the crashes yet so I think an update fixed it.
 
First thing to consider:
DISCONNECT both external monitors and run 2-3 days that way.
Do the freezes go away?

Next thing to try:
RECONNECT the displays.
IF the freezes return, you could do this:
- Set computer sleep to "never" (if it doesn't "go to sleep", maybe it won't freeze while sleeping).
- Activate a screen saver for the displays, so they will "go dark" after a set period of time, even though the computer itself remains "idle, but NOT sleeping".
- Shut it down at night and periods you'll be away from it for a while.

Does this change things?

I’ve got a similar problem with my Late 2015 iMac with two DELL monitors. I can’t remember if the iMac crashed before adding first monitor. I added a second a month later. I’ll post the report when it does it again. Usually happens overnight, not if it sleeps for an hour or so. Sometimes it just reboots on its own, or sometimes I have to do a hard re-start with the power button.

This issue happened before Catalina, and has continued with Catalina.

after unplugging the two Dell monitors, the problem went away. I will try now leaving them plugged in and turning sleep to "off," is this bad though, having computer running all the time? Is it possible the Dells are causing this and I can do something to them (ie turn on/off screen saver, or some setting) which would allow my mac to sleep without any issues? thanks everyone for your help!

Just to be clear, the iMac is freezing when you sleep or when it sleeps?
 
Get an eGPU. Months of using the eGPU with no crashes. I turned off the eGPU and ran the monitors directly with the USB-C ports and got freezing. I am back to using my eGPU for my two extra monitors.

Apps may running in the background, freeze and crash. Try to determine which apps cause the crash. You start at this: when does the crash happen? If it's when you turn on or work in a specific app, maybe you need to either reset it or remove it entirely.
 
Just to be clear, the iMac is freezing when you sleep or when it sleeps?

If I choose "sleep" manually everything is fine, it's overnight (when both me and the Mac are sleeping) that the problem happens
