In settings, I have an "empty" icon .. See below - what is this?

No clue. Did you used to have an app that started with a "0" or " "?

I have one also. Not sure if it's from an app with a name like you suggest or if an empty slot just sorts to the top. Haven't tried rebooting, etc. Upgraded with restore from ipsw and backup.

Looks like the remains of an app you recently deleted? It should go away eventually I think. Mine does that in battery usage if an app was deleted but the entry in the battery usage is still there.

I have two in my list as well. I had a couple apps that got stuck during updating - may be related to that.
