When I get my new iPhone should I get insurance for it? I usually keep my phones 2 years.
If so which kind should I get? AppleCare, carrier or another?

It all depends on your personal approach to phones and insurance in general. There isn't an answer that applies or works for everyone. As far as what type of insurance, many like AppleCare+, although it has some limitations (especially when it comes to lost/stolen phones), other insurances cover most things but with the deductible and monthly fees they might not be as worthwhile for many. Just need to look at details to see what would work better for you.

Apple Care + is best, but Apple has raised the price for it from $99 to $129 which will give you the extended coverage and accidental coverage. You might also wish to look into a third party for insurance. I've used SquareTrade in the past.

    Applecare+ is not insurance.

It's a warranty. Warranty does not cover theft or loss. And only AC+ covers external damage - at additional cost to you. The standard warranty only covers defects.
