I just got a white box replacement white iPhone 5 and when I put the Verizon SIM card in it said select the carrier. I did and it said once this choice is made it will be locked. The decision is final yadada

Verizon Sim ? Where specifically did you get that ?

Verizon doesn't use SIM cards, they use CDMA.
 
It doesn't lock the phone on GSM. It just locks the phone's CDMA/LTE to Verizon or Sprint. I have a replacement iPhone 5 (model A1429/ND209LL) and went through the same prompt.

Does it unlock it though because it didn't go away when I restored it a second time.

I haven't tried restoring my phone since I got it. I don't really care if it's locked to Verizon rather than Sprint, I would never use Sprint.

The SIM card is unlocked for GSM no matter what. All the locking does is lock the CDMA/LTE to Sprint or Verizon.

At that point you can select either Verizon or Sprint... at work I got a 4s replacement and switched to Verizon at that screen... called Verizon and activated the phone. Once you select a carrier there is not way to go back that I know.

Just restored it again and it came up again.

If you got that again you're lucky... the 4s once you select a carrier there is no way back.
