The Verizon LTE phones come with SIM cards now. You can't do LTE without a SIM card.
Just restored it again and it came up again.
It doesn't lock the phone on GSM. It just locks the phone's CDMA/LTE to Verizon or Sprint. I have a replacement iPhone 5 (model A1429/ND209LL) and went through the same prompt.
Does it unlock it though because it didn't go away when I restored it a second time.
