Would the iphones that Apple sells in their stores have iOS 15.4 already installed? Or is there normally a delay?

It all depends on how quickly they update their manufacturing process to burn new builds onto devices as they roll off the factory floor; I would imagine that any existing products manufactured around the same time as they started building the new iPhone SE, iPad Air, etc would also probably get the 15.4 GA build.

Nothing really serious. I was just curious if they somehow update the phones they have with the new iOS (unlikely, I know) or let the stock run off.
The only time I’ve ever gotten a phone with the just released software is when ordering or buying one at its initial release. Even then it may not arrive with the latest software. My 13 Pro Max came with ios 15 but 15.0.1 had already come out. No store stock will have 15.4, only recently received stock would even have 15.3.1.
