In iOS 26.2, there's a new option to set an alarm to go off when creating a reminder in the Reminders app. When adding a reminder, selecting a time and then toggling on the "Urgent" option will cause an alarm to go off at the designated time.

Alarms that you set in the Clock app can also be integrated into the Reminders app by allowing the app to access Alarms in the Settings app.

When a Reminder alarm activates, it displays a snooze option or a slide to stop option. If you snooze it, it will show a countdown on the Lock Screen that includes an option to complete the reminder or reschedule it. Tapping to complete the reminder opens up the Reminders app where you need to tap again.

In the Reminders section of the Settings app, you can also opt to remove the snooze option and replace it with a Complete button to immediately mark it as complete. Apple distinguishes Reminders alarms from standard alarms with a new blue color.

I have family members who have a horrendously long list of repeating alarms, so hopefully this can help them organize.
 
What reminders really needs is the ability to add a stop time as well as a set time for the duration. That way it will block of time on your calendar for a specific task. Now, I know some of you will just say this is an event. BUT with an event I’m not able to simply check it off my to do list in the calendar app.

This is great. I often set named timers (i.e. tell siri "set a laundry timer for 35 minutes" -- which gives you a timer with the label "laundry") for just this reason. One little notification doesn't cut it for a lot of stuff.

Something that should have been done when Reminders was created… better late than never. Will use this feature often.
 
Unironically ground breaking, I rely on Siri to set alarms purely because reminders aren’t audible enough.

It has always been such an obvious failure of the Reminders app that the reminders were way too unobtrusive.

What good is a short one-time sound that you can easily miss? The thing needs to scream until you stop it.

Only took Apple like 15 years to come to this really basic realization.

Yeah, that's just the way Apple treats reminders -- as one-off triggers without duration. Anything with a time duration is considered an event, which goes on the calendar.

For what it's worth, BusyCal will pull in tasks from Reminders just like Calendar, but will also allow you to add a duration.

You could also just add things to the Calendar, and treat the "delete" button like a checkbox. Not as satisfying as a checkbox, but it's a workaround if you don't want to use a third-party app.
