After seeing this thread tonight I noticed myself I was having the same problems. I was getting voicemails while on a phone call to my Girlfriend and noticed it never rang through. I then tried myself from my work cell phone and sure enough same problem. My phone is pwned so I was worried it was the problem so I just did a restore and still exists. I called AT&T and for some reason the call waiting, call hold, and something else was turned off! :mad: This is a free service but sure enough it was unchecked and I was unable to check "Call Waiting" from the phone preferences on my iphone settings. Make sure you have this checked if your having these problems and also call AT&T to ensure it is turned on!

AT&T Reset

Had the same problem with my 3G. After about an hour at AT&T, the tech guy discovered that there is a setting on AT&T that somehow doesn't automatically get set in the proper config. He suspected it might be more common when you port an number from another carrier (which I did). He called some magic number and told whoever to check some setting and voila. 3-way worked instantly (and ever since)

Incidently, while at the AT&T store, the tech tried all the iPhone re-sets and it didn't make a difference, so it appears to be completely an network problem.

AT&T has to activate 3-way calling

I've had the same problem that others have described. I recommend going to the AT&T tech chat (after waiting for 45 minutes on the phone, I got through faster to their chat rep). She said that my account had never had 3-way calling activated (I ported over a number from another provider), and it worked after she activated it. I had Sprint before, and I have to say that AT&T is really subpar, it's just too bad that Apple chose to partner with them. Their coverage is worse, their 3G coverage is not as advertised (gee, do you think that a 3G transmitter with a smaller radius will do just as well when you just stick them on existing cell towers?). And now they're screwing a lot of people out of their 3-way call functionality. If iPhone ever gets out of this exclusive deal, I'll be changing providers.
 
You have to call AT&T, I was unable to slide the "Call Waiting" feature to On. After the rep checked the box for my account I could slide it on but was unable to do 3 way calls until I power cycled the phone.

My Call Waiting works fine but I can't do 3-ways. The Swap button works, but the Merge button just pauses a moment and does nothing. Definitely seems like a phone system issue and not a software/iPhone issue.

iPhone 3-way calling key pad not working and drop calls

wow i feel special that im not the only one with this problem!
Sometimes it will work BUT theres another problem even when it does work!
You know how when you call, lets say, a store, and It's like press 1 for blah blah...well when I press 1 for blah blah nothing happens, and the other party in the 3 way call has to press it :( ... if anyone else is having this problem?

But with the other issue at hand with merging the calls, I still have the problem most of the time :(

I just have the same problem like yours. My mom don't speak English and I have to call her company health insurance. We were doing a 3 way calling, when the automated message ask to press 1 for English and 2 for Spanish, I press 1 on my iPhone and it didn't pick up and I have to ask my mom on the other line to press 1 for me. After we get in and waiting for the operator, the call to the insurance just died and no sound at all, but on the iPhone still show connected (Not sure why!). After a couple of tries I give up. Very frustrating.
