What video format do I choose from the list (DV, DV Widescreen, HDV 1080i, HDV 720p, MPEG-4, iSight) when importing video from iPhone?

I doubt it will be iSight or the 1080i/720p options.
It might not matter, really. Play around with them a little bit.
