My mom's iPhone 4S does this every time she records video or is FaceTiming for a long period of time. Has anyone else seen this or experience it? Should I make a Genius Bar appointment? I've restored her phone but the issue is still there.

It's not something I can replicate when I want to show someone which is why going to an Apple store & trying to get it replaced will be difficult. I can at least show them these photos.

I'd contact Apple support, see what they say. If restoring / upgrading to the newest version of the OS doesn't fix the problem, there really isn't much you can do on your own, at any rate.
