My lock/sleep button is now malfunctioning on my iPhone 5 and thought it was only working half the time but after reading your forum, it's only working when pressed on the left side just like everyone else here. Is this from my jailbreak perhaps? Will my carrier exchange my phone or do I have to go to Apple? (I'll restore my phone before doing so of course)

No, its not because of the jailbreak.
There are different contracts between apple and the carriers. Some carriers do replacements on their own, some don't and without knowing your carrier, we can't tell you. Call your carrier or AppleCare - both can tell you what to do
